Leaders from a senior-focused practice group and a skilled nursing organization come together to share how intentional collaboration has improved care coordination, reduced fragmentation, and supported better outcomes for residents. Panelists will discuss real-world challenges from disconnected systems and documentation burden to communication gaps and how closer alignment between clinicians and facility teams has led to more timely decision-making, greater efficiency, and improved satisfaction for both providers and patients. This session will explore practical lessons learned, success factors for building strong cross-organizational partnerships, and actionable strategies that practice groups and skilled nursing organizations can apply to strengthen collaboration and elevate care delivery across the continuum.

I have worked in nursing centers, assisted living facilities, and rehabilitation centers for over 15 years. I began my healthcare journey as a nursing assistant, and as I continued my education, I found this work deeply rewarding. Over the years, I advanced to new roles, first becoming a registered nurse, then working as a bedside nurse and later as a facility nursing manager. These experiences allowed me to serve in multiple leadership capacities and continually inspired my commitment to patient care. In 2018, I graduated from Walden University with a master’s degree in science, which enabled me to begin practicing as a Nurse Practitioner.

In my current position, I serve as Vice President of Medical Services for Exceptional Care Services. I am grateful to lead a remarkable team of primary care nurse practitioners who share my passion for improving clinical outcomes in long-term care. I am continually inspired by our collective dedication and the positive impact we have on the well-being of those we serve.

Gwen Sampson is a Board-Certified Adult Nurse Practitioner and healthcare executive, currently serving as Senior Practice Group Relations Director at PointClickCare. In this role, she is dedicated to amplifying the voice of practice group operators and clinicians, ensuring they have an active role in shaping the evolving healthcare landscape.

Gwen began her academic journey with a degree in Nutritional Sciences from Cornell University, later earning her Nursing degree from New York University and completing her Master of Science in Nursing at Monmouth University. As a clinical provider, she has experience across the care continuum.

 After relocating to Oregon in 2017 to work with OptumCare, Gwen transitioned into healthcare leadership, joining Ascent Healthcare in December 2019 as Director of Clinical Operations. During her tenure at Ascent, she played a pivotal role in advancing clinical operations through the implementation of new electronic health record (EHR) systems and championed the use of technology to improve care delivery. Gwen has worked to strengthen community partnerships, such as participation in the Curana Accountable Care Organization (ACO) and through grant projects with the Portland IPA. In addition, she continues to encourage active participation in the hospital system SNF Collaboratives to ensure clinicians serving the post-acute and long-term care sectors contribute in critical healthcare discussions around value based care. Her leadership and strategic vision led to her promotion as Chief Operations Officer in May 2024.

In July 2025, Gwen joined PointClickCare, where she continues her mission to elevate the role of clinicians in the post-acute care continuum and foster stronger integration between practice groups, healthcare systems, and technology solutions.

 Outside of her professional commitments, Gwen enjoys exploring the outdoors with her spouse and three children.
