Gwen Sampson

Gwen Sampson is a Board-Certified Adult Nurse Practitioner and healthcare executive, currently serving as Senior Practice Group Relations Director at PointClickCare. In this role, she is dedicated to amplifying the voice of practice group operators and clinicians, ensuring they have an active role in shaping the evolving healthcare landscape.

Gwen began her academic journey with a degree in Nutritional Sciences from Cornell University, later earning her Nursing degree from New York University and completing her Master of Science in Nursing at Monmouth University. As a clinical provider, she has experience across the care continuum.
After relocating to Oregon in 2017 to work with OptumCare, Gwen transitioned into healthcare leadership, joining Ascent Healthcare in December 2019 as Director of Clinical Operations. During her tenure at Ascent, she played a pivotal role in advancing clinical operations through the implementation of new electronic health record (EHR) systems and championed the use of technology to improve care delivery. Gwen has worked to strengthen community partnerships, such as participation in the Curana Accountable Care Organization (ACO) and through grant projects with the Portland IPA. In addition, she continues to encourage active participation in the hospital system SNF Collaboratives to ensure clinicians serving the post-acute and long-term care sectors contribute to critical healthcare discussions surrounding value-based care. Her leadership and strategic vision led to her promotion as Chief Operations Officer in May 2024.

In July 2025, Gwen joined PointClickCare, where she continues her mission to elevate the role of clinicians in the post-acute care continuum and foster stronger integration between practice groups, healthcare systems, and technology solutions. Gwen also serves on the Social Media Committee of the Post-Acute and Long-Term Care Medicine Association (PALTmed), supporting efforts to connect and inform professionals across the field.

Ian Strand, LNHA

Senior Director of Payer Relations

Ian has 18 years experience in healthcare operations, both as a provider and a payer. Ian began his career as a Nursing Home Administrator, specializing in Post-Acute Rehab, opening new facilities and improving quality outcomes. It was there that he became passionate about improving quality of care and services through aligning the goals of providers, payers, and the patient. 

In 2016, Ian took on the role of operations lead to launch and manage AgeRight Advantage, the first provider-owned I-SNP in the state of Oregon and created a medical group of Physicians and Advanced Practice Clinicians to provide on-site medical care to patients living in over 40 facilities of varying levels of care. Ian helped develop operational models and risk-based contracts that blended traditional fee-for-service requirements with the focus on superior quality outcomes and care management needed to be successful in Value-based care. 

In 2021, Ian transitioned to Patient Pattern, a technology company that provided Care Management and Clinical Documentation solutions to Payers and Provider Groups that share risk on the clinical outcomes of their members and patients. As Vice President of Health Plan Business Development, Ian provided a consultative sales approach to operators who are seeking to be successful in the world of Value-Based Healthcare. Patient Pattern was fully acquired by PointClickCare in 2023 with the goal of enhancing the tools and services provided operators in long-term care who are taking risk on patient outcomes. He now works on PointClickCare’s Value-Based Care team with the mission of connecting facility operators with payers to engage in value-based reimbursement models that reward clinical quality through better use of Medicare dollars.
