Alex Bardakh, MPP, brings deep experience in advocacy, public policy, and strategic partnerships, with a strong background in congressional and federal agency engagement. A graduate from the University of Maryland Baltimore County (UMBC) in Political Science/Psychology and Master’s Degree in Public and Legal Policy, Mr. Bardakh has extensive experience in health policy with a specific focus on areas such as payment models and quality of care initiatives. He has been a recognized national speaker on healthcare policy and has spoken at national conferences throughout the country.

Andrea Bertucci, BSN, PHN, CLHP is the founder of Bertucci Health Solutions, a consulting practice focused on value-based care strategy, quality reporting program design, and clinical operations for ACOs and geriatric practices. With 20 years of healthcare leadership experience spanning senior living, MSSP and SNP operations, clinical support program design and implementation, and population health strategy, she brings direct experience building MIPS quality reporting infrastructure that drives positive payment adjustments. She specializes in helping organizations build sustainable value-based care models for high-risk and frail elder populations.

Emily Bezold is a Senior Policy Advisor within the CMS Innovation Center. In this role, she provides technical leadership on policy development for the Accountable Care Organization (ACO) and Medicare Advantage model portfolio. Prior to this role, Emily led value-based care policy and advocacy at Bloom Healthcare, a home-based primary care organization serving complex, chronically ill homebound patients. Previously, Emily lead development of the GUIDE dementia model and served as model lead for the Primary Care First and Comprehensive Primary Care Plus models at the CMS Innovation Center.

Dr. Steven Buslovich is the Chief Medical Officer of Senior Care at PointClickCare, where he leads innovative strategies in senior care delivery. A practicing geriatrician and certified medical director, Dr. Buslovich brings extensive expertise from his role as CEO and Co-Founder of Patient Pattern—a comprehensive care management platform for value-based care that was acquired by PointClickCare in 2023.

With active involvement in key industry organizations, including the Advancing Excellence in Long Term Care Collaborative, the Society for Post-Acute and Long-Term Care Medicine (PALTmed), and the American Geriatrics Society (AGS), Dr. Buslovich is recognized for his leadership in transforming long-term care. His accolades include AMDA's Shark Tank Innovation Award, and he is currently collaborating with CMS and ONC to standardize clinical data elements across post-acute care settings.
Dr. Buslovich is an expert in operationalizing frailty risk and enhancing care outcomes within value-based care models. He continues to redefine best-practices and advocate for the future of senior care innovation.

As a physician executive with broad healthcare industry experience, Dr. Rich Feifer helps organizations achieve their fullest potential in an ever-changing environment.  Areas of expertise include population health management, value-based payment models, clinical analytics, and technology driven healthcare transformation. As a board member, senior advisor, and fractional chief medical officer, Dr. Feifer currently supports early and growth stage organizations as they navigate the healthcare system.

Dr. Feifer previously served as chief medical officer at InnovAge, a multi-state provider of PACE programs, serving frail dual-eligible seniors by delivering comprehensive patient-centric care, enabling them to age independently for as long as possible. Prior to joining InnovAge, he was executive vice president at Genesis HealthCare, one of the nation's largest skilled nursing, rehabilitation, and long-term care providers. While at Genesis, Dr. Feifer served as president of Genesis Physician Services as well as chief medical officer of the Genesis HealthCare Accountable Care Organization (LTC ACO), the first national ACO focused on nursing home patients.

Dr. Feifer’s experience extends to various boards and committees, providing strategic and fiduciary leadership.  He is a board director at PALTmed and at the Accreditation Commission for Health Care (ACHC), one of the largest healthcare accreditors. A graduate of Brown University and the University of Pennsylvania School of Medicine, Dr. Feifer is a board-certified internist with experience in population health, primary care, geriatrics, and urgent care medicine at the Fallon Clinic. He received his MPH in Health Services Management from Columbia University, and is currently an Assistant Clinical Professor at the University of Connecticut.

Dan Frascella is Chief Financial Officer of Eventus WholeHealth, a scaled provider of primary care and behavioral health services to residents of skilled nursing and assisted living facilities across a seven-state footprint in the Southeast and Midwest, and a strong performer in value-based care arrangements. Since joining in April 2025, Dan has led the company's accounting, finance, data & analytics, revenue cycle management, and M&A functions.

Prior to Eventus, Dan was Co-Founder, CFO, and Chief of Staff of WellBe Senior Medical, which he helped incubate while completing his MBA. WellBe scaled to become the largest independent provider of home-based medical care in the United States, managing approximately 150,000 Medicare Advantage lives across eight states under risk arrangements.

Earlier in his career, Dan invested in healthcare services companies at Beecken Petty O'Keefe & Company, a Chicago-based healthcare private equity firm, and advised on healthcare M&A as an investment banking analyst at McColl Partners.

Dan holds an MBA from Harvard Business School and a B.A. in Economics and International Studies from Yale University.

Ernie Ianace is CoFounder and CEO of CareAlly and CoFounder and Chief Commercial Officer of InsightAlly, a persona-based AI orchestration platform focused on healthcare, senior living, and enterprise operational workflows.

His work centers on building operational infrastructure that enables healthcare organizations to automate complex administrative and clinical coordination tasks while maintaining human oversight, auditability, and compliance within HIPAA boundaries. CareAlly supports workflows across admissions, utilization management, claims and reimbursement operations, regulatory compliance, care coordination, and workforce enablement. 

Ernie has worked with Medicare Advantage organizations, provider groups, senior living operators, ACOs, and post-acute care networks to deploy AI-driven operational systems designed to reduce administrative burden, improve value-based care performance, accelerate onboarding and decision-making, and improve cross-setting coordination.

He frequently speaks on AI governance, operational orchestration, healthcare infrastructure modernization, and the evolving role of AI in regulated healthcare environments.

Alan L. Kimmel, M.D., C.M.D. is Chief Medical Officer at Provider Partners Health Plan, where he has served since 2016. In this role, he provides executive leadership and clinical oversight for an Institutional Special Needs Plan (ISNP) serving long-term care populations across multiple states. Dr. Kimmel directs clinical care activities, establishes guidelines and protocols, and leads quality, safety, utilization management, and credentialing initiatives. As a senior member of the executive team, he contributes to strategic planning and collaborates across operations, finance, compliance, and external healthcare partners.

Previously, Dr. Kimmel served as Chief Medical Officer at Mid-Atlantic Health Care, LLC, where he oversaw clinical operations and CMS bundled payment programs for skilled nursing and rehabilitation services. He also maintained a long-standing internal medicine practice in Baltimore from 1982 to 2014.

Dr. Andrew McNamara is Senior Vice President and Chief Medical Officer at Trilogy Health Services, where he leads clinical strategy across a large skilled nursing and Senior housing portfolio with a focus on value-based care, quality performance, and population health. A board-certified family physician, he has held senior leadership roles at Aetna, MVP Health Care, McKesson, and Trust Healthcare, overseeing medical management, quality, and payment innovation. Dr. McNamara brings deep expertise in MSSP, ISNP, and ACO REACH models, and is known for aligning providers, health plans, and data strategy to improve outcomes and reduce total cost of care.

JoAnne Reifsnyder PhD, MSN, MBA, RN, FAAN has held executive and leadership roles in hospice, hospice-related medication management and skilled nursing. She shifted her clinical practice from inpatient oncology to home hospice in the early 1980’s, beginning a career-long commitment to serious illness care. Dr. Reifsnyder has held numerous organizational and board leadership positions with a focus on palliative and end of life care. She developed one of the first palliative care minor concentrations at the University of Pennsylvania School of Nursing, and most recently developed leaders through nursing and interprofessional graduate programs at the University of Maryland. She has an active management consulting and executive coaching practice, through which she works with a range of organizations and individuals to develop and lead serious illness care, age-friendly care, and complex chronic illness management services. She is a frequent speaker on leadership topics, has authored many peer-reviewed publications and book chapters, and is a co-author and editor of three textbooks.

Dr. Robert L. Russell, MD, MBA is an internal medicine and geriatric physician. He is currently Chief Medical Officer for Majestic Care and Bluegrass Consulting Group a multi state long term care provider. He is a post acute care specialist who has specialized in all aspect of long term care.

Dr. Russell is the former regional medical director for the Midwest for CommuniCare Healthcare. He is currently the chairman of the medical advisory committee for CICOA (Indiana’s largest Area Agency on Aging) and has been on the committee since its inception. He is a member of CICOA’s strategic planning committee and medical director of CICOA’s medical nutrition therapy program and a former two time president for the Indiana Medical Directors Association.

He is a member of PALTMED (formerly AMDA) and has served on numerous committees within the organization. He currently serves on the PALTMed Foundation and is a former Futures Fellow. He is former member of the executive board of the Alzheimer’s Association of Greater Indiana and completed 8 years as a board member. He is a Hospice Medical Director for Gentiva Hospice (formerly Kindred hospice). He is a 2022 graduate of the Kelley School of Business Physician MBA program. He is on the medical advisory board for Lilly and Neurocrine pharmaceutical companies and is on Neurocrine’s Delphi advisory board.

Jill Lyle Sumner is a senior healthcare executive with over 25 years of experience specializing in managed care, aging services, and health technology. As Owner and President of Lyle Health Advisors, LLC, she advises long term care providers, Medicare Advantage organizations, and health tech firms on strategy and partnership development. Jill is the ISNP subject matter expert and consultant to the SNP Alliance.

Nationally recognized for pioneering the integration of value based care models in long term care settings, Jill has held executive positions at leading healthcare organizations including AHCA/NCAL, AllyAlign Health, LeadingAge, and Virginia Premier Health Plan/VCU Health System. Her expertise spans Medicare Advantage product development, Managed Long Term Services and Supports, and population health management for long-term care providers.

Jill holds an MPH and MBA from the University of Illinois at Chicago with a specialization in Gerontology/Long-Term Care.

Originally from Omaha, NE, Dr. James “Dan” Tollman graduated from the University of Nebraska College of Medicine. He attended the Tufts University Family Practice Residency program and is a Fellow in Hospital Medicine and a Certified Medical Director. He completed his BA in Biology from Carleton College in Minnesota after which he served for five years as an active-duty infantry officer in the US Army and for four years as a medical doctor in the MA Army National Guard.

Dr. Tollman has always enjoyed practicing in a variety of clinical settings, including as a PCP, family health center urgent care, and as a hospitalist in acute care hospitals, LTACs, IRFs, and psych hospitals. Since 2012 he has been focusing on SNF care, hospice, ALF care. He has been self-employed since 2005 and was the founder of hospitalist programs in four Massachusetts hospitals (Lawrence General, Charlton, St. Luke’s, and Tobey), as well as one in Connecticut (Windham Hospital). In 2024 Dan joined his SNF practice with Curana Health as the Central and Eastern MA Area Medical Director.  He is the 2026 PALTmed Greater New England Chapter President. Dr. Tollman is an avid cyclist who is fluent in Spanish and conversational in German and recently became a first-time dog owner.

Samantha Vosloo is the Vice President of Value-Based Care Innovation PointClickCare and a Nursing Home Administrator with over 10 years of experience in Long-Term Care. She graduated from the University of South Florida and has since operated communities in Kansas and Florida. During her time as an administrator, she partnered closely with State Associations and AHCA/NCAL working in legislative activism, workforce solutions and the Quality Award Program. Driven by the goal to improve the lives of those touched by the Long-Term Care profession, she takes pride in providing process driven solutions for better outcomes.
