Prior to this role, Emily led value-based care policy and advocacy at Bloom Healthcare, a home-based primary care organization serving complex, chronically ill homebound patients. Previously, Emily lead development of the GUIDE dementia model and served as model lead for the Primary Care First and Comprehensive Primary Care Plus models at the CMS Innovation Center.

With active involvement in key industry organizations, including the Advancing Excellence in Long Term Care Collaborative, the Society for Post-Acute and Long-Term Care Medicine (PALTmed), and the American Geriatrics Society (AGS), Dr. Buslovich is recognized for his leadership in transforming long-term care. Dan Frascella is Chief Financial Officer of Eventus WholeHealth, a scaled provider of primary care and behavioral health services to residents of skilled nursing and assisted living facilities across a seven-state footprint in the Southeast and Midwest, and a strong performer in value-based care arrangements. Since joining in April 2025, Dan has led the company's accounting, finance, data & analytics, revenue cycle management, and M&A functions.

Prior to Eventus, Dan was Co-Founder, CFO, and Chief of Staff of WellBe Senior Medical, which he helped incubate while completing his MBA. WellBe scaled to become the largest independent provider of home-based medical care in the United States, managing approximately 150,000 Medicare Advantage lives across eight states under risk arrangements.

Earlier in his career, Dan invested in healthcare services companies at Beecken Petty O'Keefe & Company, a Chicago-based healthcare private equity firm, and advised on healthcare M&A as an investment banking analyst at McColl Partners.

Dan holds an MBA from Harvard Business School and a B.A. in Economics and International Studies from Yale University.

Alan L. Kimmel, M.D., C.M.D. is Chief Medical Officer at Provider Partners Health Plan, where he has served since 2016. In this role, he provides executive leadership and clinical oversight for an Institutional Special Needs Plan (ISNP) serving long-term care populations across multiple states. Dr. Kimmel directs clinical care activities, establishes guidelines and protocols, and leads quality, safety, utilization management, and credentialing initiatives. As a senior member of the executive team, he contributes to strategic planning and collaborates across operations, finance, compliance, and external healthcare partners.

Previously, Dr. Kimmel served as Chief Medical Officer at Mid-Atlantic Health Care, LLC, where he oversaw clinical operations and CMS bundled payment programs for skilled nursing and rehabilitation services. He also maintained a long-standing internal medicine practice in Baltimore from 1982 to 2014.

A board-certified family physician, he has held senior leadership roles at Aetna, MVP Health Care, McKesson, and Trust Healthcare, overseeing medical management, quality, and payment innovation. Dr. McNamara brings deep expertise in MSSP, ISNP, and ACO REACH models, and is known for aligning providers, health plans, and data strategy to improve outcomes and reduce total cost of care.

JoAnne Reifsnyder, PhD, MSN, MBA, FAAN

JoAnne Reifsnyder PhD, MSN, MBA, RN, FAAN has held executive and leadership roles in hospice, hospice-related medication management and skilled nursing. She shifted her clinical practice from inpatient oncology to home hospice in the early 1980’s, beginning a career-long commitment to serious illness care. Dr. Reifsnyder has held numerous organizational and board leadership positions with a focus on palliative and end of life care. She developed one of the first palliative care minor concentrations at the University of Pennsylvania School of Nursing, and most recently developed leaders through nursing and interprofessional graduate programs at the University of Maryland. She has an active management consulting and executive coaching practice, through which she works with a range of organizations and individuals to develop and lead serious illness care, age-friendly care, and complex chronic illness management services. She is a frequent speaker on leadership topics, has authored many peer-reviewed publications and book chapters, and is a co-author and editor of three textbooks.

Dr. Robert L. Russell, MD, MBA is an internal medicine and geriatric physician. He is currently Chief Medical Officer for Majestic Care and Bluegrass Consulting Group a multi state long term care provider. He is a post acute care specialist who has specialized in all aspect of long term care.

Dr. Russell is the former regional medical director for the Midwest for CommuniCare Healthcare. He is currently the chairman of the medical advisory committee for CICOA (Indiana’s largest Area Agency on Aging) and has been on the committee since its inception. He is a member of CICOA’s strategic planning committee and medical director of CICOA’s medical nutrition therapy program and a former two time president for the Indiana Medical Directors Association.

He is a member of PALTMED (formerly AMDA) and has served on numerous committees within the organization. He currently serves on the PALTMed Foundation and is a former Futures Fellow. He is former member of the executive board of the Alzheimer’s Association of Greater Indiana and completed 8 years as a board member. He is a Hospice Medical Director for Gentiva Hospice (formerly Kindred hospice). He is a 2022 graduate of the Kelley School of Business Physician MBA program. He is on the medical advisory board for Lilly and Neurocrine pharmaceutical companies and is on Neurocrine’s Delphi advisory board.

Originally from Omaha, NE, Dr. James “Dan” Tollman graduated from the University of Nebraska College of Medicine. He attended the Tufts University Family Practice Residency program and is a Fellow in Hospital Medicine and a Certified Medical Director. He completed his BA in Biology from Carleton College in Minnesota after which he served for five years as an active-duty infantry officer in the US Army and for four years as a medical doctor in the MA Army National Guard.

Dr. Tollman has always enjoyed practicing in a variety of clinical settings, including as a PCP, family health center urgent care, and as a hospitalist in acute care hospitals, LTACs, IRFs, and psych hospitals. Since 2012 he has been focusing on SNF care, hospice, ALF care. He has been self-employed since 2005 and was the founder of hospitalist programs in four Massachusetts hospitals (Lawrence General, Charlton, St. Luke’s, and Tobey), as well as one in Connecticut (Windham Hospital). In 2024 Dan joined his SNF practice with Curana Health as the Central and Eastern MA Area Medical Director.  He is the 2026 PALTmed Greater New England Chapter President. Dr. Tollman is an avid cyclist who is fluent in Spanish and conversational in German and recently became a first-time dog owner.

Samantha Vosloo is the Vice President of Value-Based Care Innovation PointClickCare and a Nursing Home Administrator with over 10 years of experience in Long-Term Care. She graduated from the University of South Florida and has since operated communities in Kansas and Florida. During her time as an administrator, she partnered closely with State Associations and AHCA/NCAL working in legislative activism, workforce solutions and the Quality Award Program. Driven by the goal to improve the lives of those touched by the Long-Term Care profession, she takes pride in providing process driven solutions for better outcomes.
