Approaching the New AMDA 3Ds CPG using the Care Delivery Process

This session will describe the new Delirium, Depression, and Dementia (3Ds) in Post-Acute and Long-Term Care Clinical Practice Guideline (CPG), which represents a major milestone by incorporating the care delivery process, critical thinking, and clinical decision making around these complex diagnosis. Our sessions aims to introduce the new CPG and to facilitate an interactive review of major sections of the CPG. First, we will begin with a case presentation to help generate questions. We will then focus on each of the major categories within the CPG – Recognition, Assessment, Treatment, and Monitoring – and elicit questions/answers from the audience as it relates to the next step they would take. Second, we will highlight some of the tools and resources that are available along with the new CPG to assist clinicians and medical directors in taking care of complex older adults.

  • Review the current status of delirium, depression, and dementia in PALTC medicine
  • Explore the critical thinking process associated with the care delivery process specific to recognition and assessment

Christian Bergman, MD, CMD

Christian Bergman, MD, CMD, is board-certified in Internal Medicine and Geriatric Medicine. He attended medical school at the University of Virginia in Charlottesville, VA and completed his internal medicine residency and geriatric medicine fellowship at Eastern Virginia Medical School in Norfolk, VA where he specialized in post-acute and long-term care medicine. His clinical practice focuses on post-acute and long-term care medicine with active medical directorship. He is assistant professor at Virginia Commonwealth University in Richmond, VA where he is focusing on improving transitions of care and is actively involved in teaching trainees at all levels. At AMDA – The Society for Post-Acute and Long-Term Care Medicine – he is chair of the State Based Policy and Advocacy Subcommittee and chair of the Transitions of Care Subcommittee. He lives in Richmond, VA with his wife and two kids and enjoys spending time on home improvement projects.

Nancy Overstreet, DNP, GNP-BC, CWOCN

Nancy Overstreet, DNP, GNP-BC, CWOCN, is a Geriatric Nurse Practitioner serving older adults in Assisted Living and skilled care in Central Virginia for over 20 years. Previously, an Associate Professor with Lynchburg College's Nursing Program, Dr. Overstreet returned to clinical practice with a focus on dementia care. An active member of AMDA - The Society for PALTC Medicine, she was Co-Chair of the Clinical Practice Guidelines for Urinary Incontinence and the recent Pressure Ulcer and Other Wound. She currently serves as Chair of the Clinical Practice Steering Committee.
